1. Assumption of RiskI understand and acknowledge that participating in physical activities at NHANCE Studio involves inherent risks, including but not limited to injury, physical harm, or accidents. I voluntarily assume all risks associated with the activities provided by NHANCE Studio.
2. Release and Waiver of LiabilityIn consideration for being allowed to participate in fitness activities at NHANCE Studio, I hereby release, waive, discharge, and hold harmless NHANCE Studio, its owners, employees, agents, and representatives from any and all claims, damages, or liabilities arising out of or in connection with my participation.
3. Medical ConditionI affirm that I am in good physical condition and have no medical conditions that would prevent me from safely participating in the activities offered by NHANCE Studio. If I am unsure of my ability to participate, I will consult with my physician before engaging in any physical activity.
4. Acknowledgment of UnderstandingI confirm that I have read and fully understand the contents of this waiver. I understand that by signing this document, I am voluntarily assuming the risks involved and releasing NHANCE Studio from any liability.
5. Consent for Use of Likeness give permission to NHANCE Studio to use photographs, videos, or other media of me taken during the sessions for marketing or promotional purposes.
6. COVID-19 AcknowledgmentI acknowledge that there is a risk of contracting COVID-19 by participating in fitness activities. I agree to comply with any health and safety protocols set by NHANCE Studio and acknowledge that I am participating voluntarily.
7. Governing LawThis waiver and release shall be governed by the laws of the state of Texas, and any legal proceedings arising out of this waiver shall be filed in the appropriate court within the jurisdiction of [Lewisville, Texas].
